Rejuvenation Connect ("Rejuvenation Connect," "we," "us," or "our") is a free consultation-matching service operated by Ready Patient, a brand of NW Aesthetics Medspa LLC. This policy explains what we collect through rejuvenationconnect.com and its forms (the "Site"), how we use it, who we share it with, and your choices.

1. Information we collect

• Information you give us: your name, mobile number, email, ZIP code, the skin goal you choose, when you'd like to start, and whether you'd like to book a consultation.
• Consent records: that you agreed to be contacted, the date and time, the page you used, and the number or email you provided.
• Information collected automatically: device and usage information such as IP address, browser type, pages viewed and referring source, through cookies and similar tools (see Section 7).

Please don't include medical history, diagnoses or other sensitive health details in our forms. We only ask for general goals so we can match you with a clinic.

2. How we use your information

• To match you with one participating clinic near you and pass your request to that clinic.
• To contact you by phone, text or email about your request, where you have agreed to it.
• To confirm that requests are real (for example, verifying a phone number) and prevent fraud.
• To keep records of consent and opt-outs, and to operate, secure and improve the Site.

3. Who we share it with

The one clinic you're matched with. When you submit a request, we share the details you entered with a single participating clinic that serves your area, so that clinic can contact you to book your consultation. That clinic handles your information under its own privacy practices. Any treatment, medical evaluation or advice comes from the clinic and its licensed providers, not from us.

We also share information with service providers that help us run the Site and our communications (such as CRM, messaging, hosting and analytics providers), professional advisors under confidentiality, authorities when required by law, and a successor if our business is sold or merged.

We do not sell your personal information, and we do not share it with other clinics or advertisers for their own marketing.

4. Text messages

• Consent: we only text you if you agree on our form. Consent is not a condition of purchase.
• What we send: messages about your consultation request, such as confirmations, follow-ups and reminders. The clinic you're matched with may also contact you about your request.
• Frequency and cost: message frequency varies. Message and data rates may apply.
• Opt out: reply STOP to any message. Reply HELP for help, or contact us using the details below.
• Carriers are not liable for delayed or undelivered messages.

Mobile opt-in data and consent are not shared with third parties or affiliates for their marketing or promotional purposes.

5. How long we keep it

We keep information as long as needed to handle your request, keep consent and opt-out records, and meet legal obligations.

6. Security

We use reasonable safeguards to protect your information, but no method of transmission or storage is completely secure.

7. Cookies and advertising

We use cookies, pixels and similar technologies, including tools from Google and Meta, to keep the Site working and to measure which pages and ads are effective. You can control cookies in your browser and opt out of interest-based ads through those platforms' settings.

8. Your rights

Depending on where you live, including under the Oregon Consumer Privacy Act, you may have the right to access, correct or delete your information, get a copy of it, or opt out of certain uses. Contact us using the details below and we'll respond within the time required by law. We won't treat you differently for using these rights.

9. Children

The Site is for adults 18 and older. We don't knowingly collect information from anyone under 18.

10. Changes

We may update this policy. The effective date above shows when the current version took effect.
