These Terms govern your use of rejuvenationconnect.com and its forms (the "Site"), operated by Ready Patient, a brand of NW Aesthetics Medspa LLC. By using the Site you agree to these Terms and our Privacy Policy.

1. What we do

Rejuvenation Connect is a free matching service. You tell us what you're looking for and we pass your request to one participating local clinic, which may contact you to book a consultation. We are not a medical provider, we do not provide treatment, and we don't recommend or guarantee any clinic, practitioner, treatment, price or result.

2. No medical advice

Content on the Site is general information only and is not medical advice. All cosmetic treatments carry risks and results vary. Only a consultation with a qualified, licensed practitioner can determine whether a treatment is right for you. Any care you receive is between you and the clinic.

3. Your information and contact

You agree to give accurate information and to only submit your own details. By submitting a request, you agree that Rejuvenation Connect and the participating clinic you're matched with may contact you by phone, text and email about your request, as described in our Privacy Policy. Consent is not a condition of purchase.

4. Text message terms

• Messages relate to your consultation request. Message frequency varies. Message and data rates may apply.
• Reply STOP to opt out at any time. Reply HELP for help or contact us below.
• Carriers are not liable for delayed or undelivered messages.

5. Eligibility

You must be 18 or older to use the Site.

6. Acceptable use

Don't submit false information or someone else's details, misuse the Site, try to access it without permission, or copy its content for commercial use.

7. Participating clinics

Clinics are independent businesses. We're not responsible for their services, pricing, availability or conduct. Questions about treatment or billing should go to the clinic.

8. Disclaimers and limits

THE SITE IS PROVIDED "AS IS" WITHOUT WARRANTIES OF ANY KIND. TO THE FULLEST EXTENT PERMITTED BY LAW, READY PATIENT AND NW AESTHETICS MEDSPA LLC ARE NOT LIABLE FOR INDIRECT, INCIDENTAL OR CONSEQUENTIAL DAMAGES ARISING FROM YOUR USE OF THE SITE OR ANY SERVICES PROVIDED BY A PARTICIPATING CLINIC. Some jurisdictions don't allow these limits, so they may not apply to you.

9. Governing law

These Terms are governed by Oregon law. Disputes will be brought in the state or federal courts in Multnomah County, Oregon, except where the law where you live requires otherwise.

We may update these Terms. The effective date above shows the current version.
